ERRADICACIÓN DEL HELICOBACTER PYLORI: IMPORTANCIA Y TERAPIA FARMACOLÓGICA RECOMENDADA

Resumen:  Helicobacter pylori es un microorganismo que infecta la mucosa gástrica humana. Es el agente causar de gastritis y afecciones más graves como úlceras gástrica y cáncer de estómago. La importancia de la erradicación de Helicobacter pylori es que esta afecta más del 50% de la población mundial aproximadamente 3,900 millones de personas afectada por la enfermedad, siendo un 20% que desarrollan síntomas de gastritis y otras afecciones por la bacteria. Siendo una causa infecciosa como carcinógeno tipo I. por eso la erradicación del Helicobacter pylori es la estrategia más costo efectiva para la prevención de cáncer gástrico. Existen diferentes tipos de diagnóstico para detectar la bacteria, como pruebas no invasivas, endoscopia y protocolos de biopsias, pruebas funcionales y gástricas, detención molecular de la susceptibilidad antimicrobiana, detención y seguimiento de atrofia gástrica y metaplasia intestina (prevención de cáncer gástrico). En cuanto al tratamiento hemos reportado sobre los patrones de resistencia local para una buena erradicación del Helicobacter pylori, la cuales comprenden IBP (Inhibidor de bomba de protones + uno o + más antibiótico para una triple terapia, en algunos caso una cuádruplex terapia, la resistencia de hp a los antibióticos a alcanzados niveles alarmantes lo cual repercute en la eficacia de la erradicación del Helicobacter pylori, seguir las guías internacionales para la erradicación del Helicobacter pylori recomiendan la cuádruplex terapia con o sin bismuto durante 14 días como tratamiento de primera línea en áreas de alta resistencia para claritromicina y metronidazol.

ABSTRACT: Helicobacter pylori is a microorganism that infects the human gastric mucosa. It is the causative agent of gastritis and more serious conditions such as gastric ulcers and gastric cancer. The importance of the eradication of Helicobacter pylori is that it affects more than 50% of the world population, approximately 3.9 billion people affected by the disease, with 20% developing symptoms of gastritis and other conditions caused by the bacteria. Being an infectious cause as a type I carcinogen, that is why the eradication of Helicobacter pylori is the most cost-effective strategy for the prevention of gastric cancer. There are different types of diagnosis to detect the bacteria, such as non-invasive tests, endoscopy and biopsy protocols, functional and gastric tests, molecular arrest of antimicrobial susceptibility, arrest and monitoring of gastric atrophy and intestinal metaplasia (prevention of gastric cancer) regarding the treatment, we reported about the local resistance patterns for a good eradication of Helicobacter pylori, which include PPI (Proton pump inhibitor + one or more antibiotic for a triple therapy, in some cases a quadruplex therapy, the resistance of HP to antibiotics reached alarming levels which has an impact on the effectiveness of HP eradication, following the international guidelines for Helicobacter pylori eradication recommend quadruplex therapy with or without bismuth for 14 days as first-line treatment in areas of high resistance to clarithromycin and metronidazole.
